How to fill application

Note :

Fields mark with * are compulsory to fill.
Provide applicant's E-mail Id exactly in the way as it will have to be used while sending the mail.
You must have the Demand Draft ready before filling up the application form. Application will not be submitted without the details of the Demand Draft.

First of all applicant has to select the Stream for which service (Duplicate Marksheet/Duplicate Certificate/Migration Certificate/Duplicate Marksheet with Name Change) he/she is applying.

Enter personal details of student like Surname , Name , Father's Name etc.

Fill phone number, if available.

Enter mobile number it is compulsory.

Enter student's E-mail ID. Please note that all the any communication will be sent on this email address. It is advised that you provide correct and valid e-mail ID.

Then he/she has to give his examination Details , first enter Exam Seat Number then Exam Month from its dropdown list , after selecting month select Exam Year properly .
After entering these details select the school Index Number as shown above.

Select the service for which student wants to get required certificate/Marksheet etc.Please note that student can apply for more than one service at a time.

To get the documents by post enter your address to which all communications should be sent. Please note that it is mandatory to provide the PIN Code. Address without PIN code will be submitted .
To provide complete address please select district , taluka and city name respectively from their list .

If student wants to get the document by post additional postage charge will be added in application fees.

Select "NO" if do not want to receive documents by post. On selecting NO you don't need to enter address details. In this case student has to collect his/her document from GS&HSEB.

There are three ways for payment in application .Select the Mode from list by which you are paying Application Fees.

After selecting mode provide the payment details like DD Number, Date and Bank Name, Bank Branch Name and MICR Code.
Now , Fill different application details according to your selected service.as in above image.
